Certain items are not eligible for grant funding, such as home modifications, shower installations, and accessible vehicles, 
An incomplete application will not be considered. 
Your application must include:
         Photo of yourself.  Your Bio/Story.  Quote/Invoice including all specifications from 2 vendors/providers.
